The only code of dueling that is set in stone, is that the duel must be agreed to by all parties involved. Any other provision of dueling is completely at the discretion of those involved, an thus, any attempt to codify formal terms is subjective, and should be viewed as such.
There are standard terms of dueling, those standard being set by the duelist themselves. While a duelist can request any terms he wants, it does not qualify as precedence until such terms have been accepted in at least one duel. Even then, a single precedence is not necessarily a standard. By the time a given term has become well known and commonly applied, by active duelist, it effectively becomes a standard of dueling. Since the norm are dictated by averages, the code will vary over periods of time.
I am only one duelist of many, and I can only present my own observations.

Typical Standards of Dueling.
No outside interferences.
No running.
No reloading/repairing.

This is not a comprehensive list. I have offered and/or agreed to many different terms at one point or another, many of which should never be suggested as a standard. It isn't really a standard until it becomes common usage. What may be considered common is often swayed by duelist who duel many other duelist, as they spread the precedent across many duelist, and those duelist may adopt that standard.

It isn't that there isn't a standard of dueling. It is only that the standard is a product of the participants, and thus somewhat dynamic.

The only standards truly adhered to in a fight are the standards that each pilot holds himself to. Just my view of course.
